Diabetes Health Education Program
If you have diabetes, you know that managing your disease plays a
significant part in your life. Our Diabetes Education Program,
available to Arkansas Blue Cross and Blue Shield, Health Advantage
and eligible BlueAdvantage Administrators of Arkansas members,
is a free, program designed to provide you with the latest
information on managing your disease.
This information can help you prevent common complications of
diabetes and quickly and effectively resolve acute episodes of
illness associated with diabetes.
Program Highlights
- Newsletters and reference materials mailed to you on all
aspects of diabetes care, including: diabetes test and medication
tracking cards, menus and recipes, information on the importance
of regularly scheduled care (eye exam, hemoglobin A1c, lipids,
foot exams) and occasional incentive items such as a pill box, a
pedometer, and a t-shirt when you complete the program.
- Voluntary Health Survey: A comprehensive questionnaire
by telephone will help determine your current health status and
identify your health education needs. A personalized report will
be mailed to you afterward.
- Member Contact: Our registered nurse case managers will contact
you to help you find local and national resources. They also can
explain your health-plan benefits.
- Physician Collaboration: We will help you work with your
physician to obtain diabetes information using the American
Diabetes Association Standards of Care and your physician's
care plan.
- Health-Care Resource List:
This comprehensive list can help you locate information on the Web,
community resources and classes on diabetes care and education.
- Satisfaction Surveys: We want to know how the program is
working for you; on occasion we will ask you to fill out a survey.
Physician Support
When you join the Diabetes Education Program, your physician
remains your caregiver and he or she will continue to makes all
medical decisions. The Diabetes Education Program provides health
education assistance to you and your physician in managing your
disease through information, benefits management and coordinating
access to local and national resources.
Waived Deductible on Testing Supplies
Arkansas Blue Cross recognizes the benefits of routine
blood sugar testing for people with diabetes. To encourage our
members with diabetes to continue this important activity, we will waive the deductible for blood glucose testing supplies (lancets and testing strips) for Arkansas Blue Cross Blue Shield members who are currently, or were previously, enrolled in the adult Diabetes Education Program or the On The Level Youth Diabetes Education Program (for children and adolescents ages 17 and younger).
Other Health/Cost Advantages
If you are dealing with the effects of diabetes, you know firsthand what the cost
is to your health and quality of life. The financial costs are another issue. Better
health leads to a better quality of life and lower costs to you. If you are a BlueAdvantage
member, check your member benefits document for coverage of services.
